Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 11/18/2025, and are subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Surveyor observed 2 activity areas (of 2 areas) and found that the areas were adhering to group size requirements at the time of the inspection. The outdoor area's resilient surface was extremely wet and unable to be inspected for resilient surface requirements at the time of the inspection

Initial Comments: No deficiencies were observed at the time of the compliance inspection conducted on 11/19/2024 and are subject to change pending programmatic review. Compliance Officer: J. Anderson The Compliance Officer reviewed 3 staff files. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. The following assessments were completed not DES Group Size Evaluation Empower Program Assessment was emailed to the director.

Initial Comments: The following deficiency was observed at the time of the Compliance inspection conducted on 12-18-2023 and are subject to changes pending programmatic review. State Compliance Officer 1: Joel Guerrero State Compliance Officer 2: Jamie Anderson A full inspection was conducted at this time. 3 of 3 fingerprint cards were verified to be valid via the DPS website during the time of the inspection. The Empower Survey was completed after the time of the inspection. The DES group size checklist was not completed at the time of the inspection.
