FACTS--Family and Community Teaming for Students, provides a safe and drug-free community-wide before and after-school program where children have access to a broad range of activities, including sports, recreation, academic and enrichment opportunities. FACTS operates in the District's elementaryschools and is open to children ages 5 to 12, whether they attend public, private or charter schools, or are home-schooled.

There were 3 staff interviewed during the investigation. There was 1 staff files reviewed during the investigation. This was a self-reported incident and the F.A.C.T.S. Director was interviewed. The Flagstaff Police Chief was also contacted. Documentation reviewed included the following: Flagstaff police report. Upon completion of the case investigation, it was determined from observation, interview and documentation that the allegations were substantiated.
