Findings: Based on a review of the facility's video, the Surveyor observed that the health and safety of an enrollled child was placed at risk of harm when a staff member slapped a child's cheek for biting another child. A staff member was on their cell phone with their back to the children. The staff turned around when they heard children crying and noticed child 1 was attempting to bite child 2 on the arm. The staff walked over to the children and slapped child 1 on the cheek with an open hand.

Findings: Per allegation and a review of the facilty's video, the Surveyor observed that a staff member failed to supervivise enrolled children when a staff member had their back to a group of 1 year old children. This staff member was the only staff in the room. The staff member was looking at their telephone and was not supervising the children. During this time, a child was attempting to bite another child.
