About the Provider
Pinecone Preschool's mission is not only to provide daycare and childcare in Flagstaff, it is also to provide the families in our community with an early childhood education program. This program develops social, emotional, physical, and intellectual growth through creative play and loving instruction in a family environment.

| Initial Comments: The purpose of the investigation was to conduct Complaint # 00169756 investigation on 6/2/26 A full inspection was not conducted at this time. A focus inspection was conducted The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Ratios observed were: 1:2 1yrs 1:3 infants 2:3 1yrs 3:8 infants 2:11 3 years and up 3:12 2 years 1:10 4 years 1:11 4 years There were 2 staff interviewed during this investigation. There was 1 staff file reviewed during this investigation. The Compliance Officer attempted to contact the complainant on 5/19/26 by email and was not able to make contact. Documentation observed was DPS clearance card documentation and through the DPS website. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 1 allegation was unable to be substantiated due to a lack of sufficient evidence. There were no deficiencies found. This is subject to change pending programmatic review. During the exit interview, the following items were discussed, but were not limited to: Staff file requirements and supervision of non staff members while they are on premises during the hours of operation. | |||

Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 7 of 7 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Surveyor observed 10 activity areas (of 10 areas) and found that the areas were adhering to group size requirements at the time of the inspection.
