The staff/child ratios for children, two years and older, during naptime were not maintained by having at least one person in each room, visually supervising all children and the total number of required staff on the premises within calling distance of each room occupied by children. Ten, one-and two-year old children were monitored with one caregiver in their section/space in the gym. An additional caregiver on the other side of the gym was asked to come to the toddler/two's section to maintain nap time required ratios.

Each child's diapers were not changed at areas designated exclusively for diapering, on a surface that was smooth, nonabsorbent, easily cleanable and of tight construction. Ten, one-and two-year old children in diapers and pull-ups were diapered in a bathroom without a using a surface that is smooth, intact, nonabsorbent, easily cleanable and of tight construction. (Changing mat was not present nor used by staff when diapering children)

Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Two bottles were monitored not labeled and dated.

A safe indoor and outdoor environment was not provided for the children. Two bags of fertilizer were monitored stored on the ground and next to the children's garden. The label was read and it was monitored with multiple warnings. The two bags were removed and stored under lock and key during the visit.

The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. Weeds were observed growing through the mulched areas and around the fence border.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space #2, push pins were monitored used on posted items directly over the infant cribs. The push pins were removed during the visit.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Approximately 4-6 unused electrical outlets were not covered. The sockets were covered once identified during the visit.
