All walls and ceilings including doors and windows were not kept clean and in good repair. In space#3 the door and walls have come peeling paint, and sticky patches from tape and connect paper.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Three outlets were uncovered.

Air conditioning units were accessible or did not have a guard to keep objects from being thrown into the unit. The air condition gate has a bicycle lock on the gate; however, I was able to push open the gate and walk directly into the area to have access to the air conditioner.

Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. In Space #2 one child had an EPI Pen, there was no Medical Action plan for the EPI PEN.
