Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last annual Fire Inspection the facility has on file was conducted on 01/08/24. However, this information was not shared with the consultant within seven days of completion, as required.

Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. In Space #3 broken plastic traffic signs were observed accessible in the Block area. I explained to Ms. Waddell that these items posed a safety hazard, as they are in poor repair, and would need to either be repaired or discarded.

A safe indoor and outdoor environment was not provided for the children. The outdoor learning environment was monitored. It was observed that the chain link fence is beginning to separate in the back left corner and there is an opening about 10 inches wide. It was also observed that there is visible erosion at the rear of the playground creating a tripping hazard for children and these need to made inaccessible to children until they are repaired.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. It was also observed that one new staff member hired on February 20, 2024 did not have a completed emergency information form on file.

Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. Seven (7) children’s files were monitored today, and it was observed that three (3) children had incomplete transportation forms on file.

Seven (7) children’s files were monitored today, and it was observed that one (1) child did not have a completed application on file indicating whom the child is authorized to be released to.

A valid qualification letter was not on file and available to review at the facility. It was also observed that one new staff member hired on February 20, 2024 did not have a current criminal background check on file.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. It was also observed that one new staff member hired on February 20, 2024 did not have a completed emergency information form on file or a signed statement acknowledging review of the facility’s shaken baby policy prior to caring for children on file.

Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. It was also observed that the health information and medical assessment for the new staff member hired on February 20, 2024 was stored in a folder with all other employee paperwork and not in a separate folder, as required.

Staff did not complete the health and safety training within one year of employment. It was observed that two veteran staff members had not completed the required Health and Safety trainings within the first year of employment, as required.

A violation was found for which there is no item number. 15A NCAC 18A .2826 LIGHTING AND THERMAL ENVIRONMENT (b) All rooms used by children shall be heated, cooled, and ventilated to maintain an ambient temperature between 65 degrees Fahrenheit and 85 degrees Fahrenheit. It was found that children were in attendance for approximately 3 hours and the facility's temperature was below 65'F.
