Findings: The rabies for the provider's dog expired in February 2023, this will need to be updated ASAP. Please send a copy of the rabies certificate to the licensing specialist once received but no later than June 2, 2023
2022-04-20 Mandatory Review 13A.15.10.02 Corrected
Findings: The provider reports that there are firearms in the home that are stored in the workshop room and the ammunition is stored in a separate storage room. In addition, this licensing specialist observed a rifle on a shelf above the provider's son's bed and she stated it is a ceremonial rifle that he was given by the Eagle Scouts (they are currently waiting for a display case to keep it in). His bedroom is an approved child care area. The provider will need to remove the rifle from above the bed and store in a locked cabinet. The provider was not sure exactly where the firearms were located in the workshop room and this licensing specialist attempted to locate them in the workshop room but was unsuccessful. She will need to send a picture of where all the firearms are stored and show that they are locked up along with a picture of the ammunition that is stored and locked up. Once the provider receives the case for her son's rifle, as long as it either sealed or has a locking mechanism it may be placed back on the shelf for display. Please send pictures no later than May 20, 2022 to the licensing specialist.
