Findings: There was one emergency form that was completed in April 2022 but there have been no updates since, this emergency form needs to be updated immediately. There are 2 emergency forms that need to be updated no later than May 2024. There was one emergency form that was not dated by the parent but the child has not been enrolled longer than one year, this parent will need to date the emergency form. Please send copies of the corrected/updated emergency forms to the licensing specialist immediately.

Findings: There was one child that did not have the Health Inventory II or a lead blood test result, two children that did not have evidence of the lead blood test and three children that need updated immunizations. See remarks section for details. These item are due to the specialist immediately.

Findings: The provider contacted the Office of Child Care on April 23, 2024 to report that her daughter has moved back in temporarily, along with her boyfriend and their infant child. The regulation states the provider must notify the Office of Child Care immediately of change in residents. The provider indicates that they moved in on April 12 and it was 7 business days before the provider contacted the Office. The provider's daughter is an approved substitute so clearances on her are on file. This Licensing Specialist received the notarized release of information for the boyfriend at the inspection. This office has also received the criminal background clearance for the boyfriend. The provider will need to submit a medical evaluation for all new residents in addition to the other medicals required for renewal. The new resident medicals are due now and the remaining medicals are due by June 30, 2024.

Findings: The water temperature in the bathroom measured at 129 degrees Fahrenheit; the water temperature will need to be reduced so it is not higher than 120 degrees Fahrenheit. The provider stated she will have her husband look at the water heater once he gets home and reduce the temperature. Please measure the water temperature after an appropriate amount of time and send a picture of the temperature to the licensing specialist.

Findings: The provider has an above ground pool and at the last inspection this licensing specialist discussed the requirements of the need for a fence surrounding the pool. At that time the provider talked about putting a deck around the pool instead of a fence so options for decking were discussed. The provider did not install a deck around the pool and there is no fence surrounding the pool. The supervisor, Joy Robertson, was consulted during this inspection. The provider will need to install some type of barrier around the pool or come up with an alternative option and submit the plan to the Office of Child Care for approval. There also needs to be an alarm in the pool and the alarm on the access door needs to remain on during child care hours. This needs to be corrected immediately. Follow up visits will most likely be made to monitor compliance.

The provider will need to inform the LS how she will be sure this happens from this point forward.
