Findings: The outdoor play area, which is located in the fenced-in backyard posed multiple health and safety risks. When entering the backyard from the walk-thru basement, is a wooden plank that appears to be weak and spongy from water. The provider stated she would be removing this platform. There were overgrown vines and possibly poison ivy/oak growing along the perimeter of the fence. The grass had not been maintained and vines were growing on the ground. There was an electric cord hanging over the fence. The other end of the cord was hanging over the fence onto the neighbor's property. This area is not suitable for children to play due to these safety hazards. It was agreed that the outdoor play area would not be utilized for play until the area is corrected. The provider agreed to take the children to the local playground which is located across the street. The Licensing Specialist discussed with the provider about the requirement of maintaining safe outdoor play space. The provider has received other noncompliances for concerns regarding the outdoor play space at previous inspections. Ensure regulations are met at all times. Photographs were taken of this area.

Findings: At the bottom of the steps leading to the basement, is a hazardous area. There was a platform that was at the bottom of the steps that was removed for repairs. This platform has not been replaced; therefore, there is a large opening that is fall hazard. This is a danger for individuals trying to utilize the steps. Due to this hazard and the hazards on the playground, the provider will utilize the local playground for outdoor play. The basement is only approved as a walk-thru to the outdoor play area. The provider stated the landlord agreed to correct the platform area by 7/18/2019. Photographs were taken of this area. The Licensing Specialist will conduct a follow-up inspection.
