One employee did not have a current Emergency Information form on file today. The employee completed a new form during my visit.

All stationary equipment, more than 18 inches high, was not installed over protective surfacing. On the playground for school age children, I observed the loose mulch surfacing to be two inches deep under the stationary climbing equipment. The mulch had become deteriorated causing it to be compacted.
