The girls bathroom on the left hallway by the cafeteria had no paper towels and the girls bathroom on the right by the gym had no hand soap at one (1) out of the three (3) handwashing sink. The boys bathroom on the left hallway by the cafeteria had no hand soap at two (2) out of the three (3) handwashing sinks.

In Space 6, I observed three (3) containers of hand sanitizer stored below five (5) feet, one (1) container of Mr. Clean cleanser stored in an unlocked cabinet space, and one (1) container of disinfecting wipes stored on an open bookshelf. On Playground 1 outside the cafeteria, I observed one (1) slide broken at the top and the bottom of the slide. On Playground 1 outside the cafeteria, I observed one (1) spinning seat device broken in several places on the seat where it attached to the descending pole.

Parent's medication authorization did not include required information. Two (2) Medication Administration Permission and Record forms had no valid dates for permission listed on the forms.

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Photographs were missing on the child emergency information for all the children enrolled.
