Findings: Based on interviews with staff and children, it was determined that during hand washing and remote learning, Staff 1 has been overheard saying "you should know how to wash your hands," "not like that," and "get your life together" in a loud and harsh voice that does not model and encourage orderly conduct.

Findings: Upon inspection of the diaper changing area in the two year old activity area, Surveyor 1 measured the water temperature of the hand-washing sink next to the diaper changing surface to be 78. The water temperature was adjusted during the inspection.

Findings: Upon review of the facility's sanitation permit, Surveyor 1 noted that the permit expired on 11/30/2019. The facility director provided Surveyor 1 with a copy of a receipt which verifies that the permit fee was paid to the Pima County Health Department on 2/4/2020.

Findings: Upon review of the facility's enrolled children's medications, Surveyor 1 noted that the medication for Child 5 was prescribed as a two pack, however, there was only one of the two medications in the box. Staff 1 acknowledged that the facility did not have written authorization from the child's health care provider to allow the medications to be separated.

Findings: Upon inspection of the outdoor activity area, Surveyor 1 measured the wood chips in the fall zones of the playground equipment as follows: -the wood chips in the fall zone of the chain ladder with blue rails measured 2.5"; -the wood chips in the fall zone of the staircase leading up to the climbing equipment measured 4"; -the wood chips in the fall zone of the red, spiral slide measured 1.5".
