The following information was missing: * Child 1 - The emergency record was not signed. * Child 2 - Dr. name and phone number, and the name of the individual to be contacted in case of injury or sudden illness of the child. * Child 3- Dr. name and phone number, and question #4 on back the emergency record was left blank.

Findings: * Surveyor 2 observed that Child # 4 did not have a copy of their immunization record.

Findings: * Surveyor 1 observed 8 enrolled children go to the playground with 1 staff during the inspection. A few minutes later, Surveyor went outside where the children were playing in order to inspect the playground. Surveyor asked the staff where the children get their drinking water. The staff stated that each child brings out their own water. Surveyor asked each of the children if they all had water; 2 of 8 children stated that they did not have water. The group remained playing outside. The Surveyor returned to the classroom and informed the Director that 2 children did not have drinking water outside. The Director radioed the other staff to come back into the classroom so that the children could get their water bottles. According to accuweather.com, the high temperature on the day of the inspection was 99 degrees Fahrenheit.

Findings: * Surveyor 1 observed several bottles of sunscreen being stored in individual gallon size bags. The bags were labeled with the children's names, not the actual bottle. Surveyor observed one bottle of sunscreen that had fallen out of the bag and was not labeled.

Findings: * Upon entering the main classroom there is a table where the sign in and out binders are located. Surveyor observed several plastic baskets storing the children's backpacks and lunch boxes lined up from one side of the table to the designated exit. There was not a clear route to exit the classroom from both sides of the classroom as the baskets were obstructing one side of the exit. (a picture was taken)
