Description: The KidzCo Early Learning Center provides child care for children ages one month to 12 years at three different locations in Tucson, Arizona. Our learning center is clean and safe for your children, with a staff of trained professionals with more than 10 years of child care and educational experience. |Findings: The Surveyors noted that the facility is using electronic versions of the Emergency, Information and Immunization Record (EIIR) cards for the emergency binder. When viewing the electronic versions, Child 1 did not have a record, and the records for Child 2 and Child 3 were corrupted, and not able to view.|
|Findings: Based on review of sign in and sign out documents Surveyor 1 noted the following: -Child 4 was signed out without a signature on 11/05/2020, and was signed in without a signature on 11/17/2020. -Child 5 was not signed in on 11/03/2020. -Child 6 was not signed out on 11/11/2020 or on 11/12/2020.|
|Findings: Surveyor 2 noted that Staff 1's file only documented contact with one reference upon hire.|
|Findings: Surveyor 2 noted that Staff 2 completed six of the required eighteen or more hours of annual training between the period of 12/19/2018 and 12/19/2019.|
|Findings: There were three staff members on the premises when Surveyors first arrived, and each one was needed to maintain staff-to-child ratios in the three classrooms, leaving no additional staff onsite to perform administrative duties, food service, cleaning, etc. Upon arriving, the Surveyors had to call the main facility phone in order to get the security code required to gain access from the office into the main hallway, as nobody was available to open the door for Surveyors. Additionally, the Surveyors were not able to gain access to some of the licensing documents that were located in the front office until a fourth staff member returned from their break.|
|Findings: Surveyor 1 noted that the posted lesson plan in the Two's Classroom was not for the current week, it was dated for the week of November 08, 2020.|
|Findings: Based on review of EIIR cards Surveyor 2 noted the following: -Child 7's modified diet was not posted as required. -Child 8's modified diet posting included only 2 of the 3 items documented on their EIIR card.|
|Findings: Surveyor 1 noted that the sand within the fall zones of the tan slides did not meet the minimum 6 inch depth. A measuring tool was used to verify that the sand was 2 inches deep.|
|Findings: Surveyor 1 noted that there was an unused electrical outlet that was not covered near the hallway door in the Two's Classroom.|
|Findings: Surveyor 1 noted that exhaust fan switch is not accessible for the restroom located in the Two's Classroom, it is only accessible from the restroom located in the Classroom next door.|

|Findings: Based on review of staff files the Surveyor noted that Staff 1's file did not contain a copy of their fingerprint clearance card as required. The Surveyor was able to verify via the DPS website that the card was valid during the inspection using a copy of the application that was in the file.|
|Findings: Based on review of fire drill documents Surveyor 2 noted that a fire and emergency evacuation drill was not conducted for the month of December 2018.|
