Findings: The facility 's documentation of their most recent gas inspection was dated 3/17/2020 and was not completed within the previous 12 months. On 3/26/2021, the facility provided documentation to verify that a gas inspection was conducted on 3/23/2021.

Findings: The Team Leader reviewed two staff files and noted that the file of Staff 2 was lacking their educational documentation. Staff 2 left the facility and returned with the documentation prior to the completion of the inspection.

Findings: Per the allegation, based on staff interviews and Surveyor 1's observation, the drinking water containers in both the toddler and two year old activity areas were on shelves that were too high for children to reach. The teachers in both activity areas reported that water is offered at different times of the day or given to children when they ask for it. Furthermore, staff members reported that when 1 and 2 year old children are outside, water is not within children's reach, but is given to children when they ask for it. It was also reported that 1 year old children have been required to sit down when drinking water so that they don't spill it.
