Plan: This letter is to formally document the corrective actions taken in response to an incident involving a staff member who struck a child with a shoe while in care at Kreative Kidz Learning Academy. Immediately upon learning of the incident, the staff member involved was terminated from employment. The safety and well-being of every child in our care is our highest priority, and we maintain a zero-tolerance policy for any form of inappropriate discipline, abuse, or neglect. An internal review was conducted promptly, and all required reporting procedures were followed in accordance with Oklahoma DHS Child Care Licensing requirements. We fully understand the seriousness of this matter and have taken the following corrective measures to prevent any future occurrence: 1. All staff have completed additional training on child abuse and neglect recognition, prevention, and mandatory reporting requirements. 2. Refresher training was conducted on active supervision practices, appropriate discipline policies, and maintaining professional conduct at all times. 3. Staff were reminded that if they feel overwhelmed or unable to perform their duties appropriately, they are required to immediately notify administration and may be relieved from duty for the remainder of the day. 4. Additional training was provided regarding proper use of classroom materials and equipment in accordance with manufacturer guidelines and safety standards. 5. Supervision monitoring procedures have been reinforced to ensure consistent accountability and oversight. Description:
Program does not have a qualified director at this time. Previous director left on 10/23/25. Licensing requested personnel information sheets for 5 staff during the visit. Staff in charge reported that the staff have been employed over two weeks. Staff could not find the forms during the visit. Licensing observed a bottle in the crib with a sleeping one year old. Licensing observed the toilet to be removed from the bathroom in the middle classroom and sitting on the shelf. Staff reported they were having it replaced due to it not working. Licensing observed the ground to be tore up in the outdoor play area, creating large holes, and exposed piping. Licensing observed a shovel next to the holes. |
