In the bathroom, used by the children 2 years of age, there was sharp broken plastic on the divider wall for the toilets where the old toilet paper dispenser used to be. Under the roof of the shade structure, there was a wasp nest. In the drawer on the far right in the kitchen, there was a container of supper glue.

In space 1, plastic bags were used to store snacks, diapers, and cups in the Ready to Go box. The box was on the floor in the classroom and accessible to the children.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. On the playground, the protective cover on the electrical outlets on the outside of the building had fallen off making the outlets accessible to the children when they are on the playground.

Three staff members did not have documentation available for review that the required trainings for their 5 year cycle had been completed.
