In space 1, children's hands were not washed after touching the rug in the center time space and before eating lunch. Children's hands were also not washed after lunch and before resting on their cots.

On the incident reports reviewed, 5 did not have a parent signature, 6 did not include the date of the parent signature, 3 were not signed by the provider who completed the form, and 5 did not include the name of the person who contacted the parent.

Enhanced staff/child ratios and group sizes were not met. There were 13 children in care in both classrooms with 2 staff members in each class. The youngest child in both classrooms was 3 years of age making the staff/child ratio 1:10 at enhanced standards. When I arrived, a staff member from space 1 left the classroom to set up space in the office for me to work. Later, one staff member from space 2 left the classroom to take the food cart to the door in space 1. A provider from space 2 also took 1 child to the restroom located in space 1. To provide one-on-one teaching, a provider took a child from space 2 to space 1 while the children in space 1 were on the playground.
