One child, HD, had a current medical action plan on file for Diazepam which was signed by the doctor. The form did not include a statement giving permission to administer the medication and was not signed by the parent. No other documentation of permission to administer the medication was on file.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In the bathroom used by children 2 years of age, there was a plastic trash bag filled with children’s shoes located in the cabinet under the sink. In the teacher’s desk drawer in space 1, there was a plastic zip lock bag with card board shapes. There were children under the age of 3 in care in this space.

Center has not passed required sanitation inspection and received an approved or superior rating. An inspection was not completed annually. An inspection was completed on 2/10/22 however the current inspection was not completed until 4/19/23.

Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. The pharmacy label for one child's Diazepam stated that the medication should be discarded by 10/7/2022. The sealed plastic box of medication was marked with an expiration date of 7/2026 and did not match the pharmacy label.

A caregiver administered medication to a child without parental permission or authorization from a bona fide medical care provider during the event of an emergency medical condition when the child's parent was not available. The medication was not administered, however, in space 3, there was a permission form for one child's albuterol inhaler that was completed on the form used for topical ointments and dated 9/7/22 to 9/7/23. Permission for this medication must be renewed every 6 months and permission expired on 3/7/23.
