All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Four staff hired between April 28, 2025 and January 12, 2026 had not completed First Aid training. Documentation of First Aid training, completed on January 27, 2025 was not available for a staff person hired on November 1, 2024. First Aid training completed by a staff member on February 16, 2024 expired on February 16, 2026 and has not been renewed.

Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Two staff members have not completed their five year criminal background checks which expired February 11, 2026 and May 24, 2026.

Medical exam or health assessment record was not on file before or within 30 days after admission. In one of six children's files reviewed, the children's medical report was not received until October 3, 2025 for a child enrolled on May 5, 2025.

Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. In one of six children's files reviewed, immunization records were not received until May 26, 2026 for a child enrolled on March 30, 2026.

A valid qualification letter was not on file and available to review at the facility. Valid qualification letters were not on file for two staff members whose qualifying letters expired on February 11, 2026 and May 24, 2026.

A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Aquaphor diaper cream, Think Baby cream, Babo Botanicals cream and Aveeno sunscreen kept on site for children did not include written authorization forms from the parent/guardian.
