A safe indoor and outdoor environment was not provided for the children. On November 9, 2023, a staff member failed to ensure a safe environment when she moved a bottle from a bottle warmer without wiping the bottom of the bottle. The bottle was placed on a shelf and then carried across a child's body and hot water dripped on the child and caused harm. The child received a second degree burn on the leg.
