Noncompliance Area: A cracked plastic shopping cart was observed in an area accessible to children in care in the outdoor playspace, which presents a threat of laceration and/or puncture injury.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The file for Child #1 included an emergency contact form that did not include written parental authorization for consent for emergency medical care.

Correction Required: Emergency contact information must include the written consent signed by a parent for emergency medical care.

Noncompliance Area: The files for Child #1 and #2 contained immunization records, however, neither record included documentation of a flu vaccine in the past 12 months.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: The file for Child #1 included an emergency contact form that did not include parental written consent for the administration of minor first-aid by facility staff members.

Correction Required: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Noncompliance Area: Although the file for Staff #1 contained documentation of the completion of Mandated Reporter training, said training was completed more than 60 months ago.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: Although the file for Staff #2 included current PA State Police and DHS Child Abuse clearances, the previous clearances for each were completed more than 60 months ago.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: Near the base of the wall near the entrance to the facility, in an area accessible to children in care, a cracked plastic electrical outlet cover was observed to create the potential for laceration and/or puncture injury.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Cracks with jagged edges were observed on three ride-on plastic vehicles, a plastic weed whacker toy, and an approximately 4' high plastic rail road sign in the outside play space in areas accessible to children in care. Cracked edges could cause laceration injuries.

Noncompliance Area: The file for Child #1 contained an agreement that did not include the persons to whom the child can be released.

Correction Required: An agreement shall specify the persons designated by a parent to whom the child may be released.

Noncompliance Area: The file for Child #1, an approximately 3 year and 4 month old child, did not contain documentation of two Hepatitis A vaccines as recommended by the ACIP. The file for Child #2, who is approximately 20 months old, included documentation of immunizations; however, the following immunizations were not documented and are recommended to have been completed by the ACIP: two Hepatitis A, 1st Varicella, 3rd HIB, 4th PCV, 4th DTAP, and 1st MMR.

Noncompliance Area: A fence post on the vinyl picket fence enclosing the outside play space was observed to have an approximately 1"x2" piece of sharply-edged vinyl loosely attached near the top accessible children in care, which could cause laceration injury

Noncompliance Area: An orange traffic cone located in the outside play space, in an area accessible to children in care, was observed to be cracked near the top of the cone with exposed, jagged plastic.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The file for Child #1 contains a health assessment that was completed more than 60 days after the child was enrolled in care.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: The file for Staff #1 contains a Pennsylvania Child Abuse clearance that expired on 07/13/2018. Said file does contain documentation that an application and payment to obtain said clearance was submitted on 07/30/18. Although the PA State Police and FBI clearances for Staff #1 are current, they were completed more than five years after previous clearances. Current FBI clearance for Staff #1 is dated 08/01/18; the previous clearance expired on 06/13/18. The current PA State Police clearance for Staff #1 is dated 07/30/2018; previous clearance expired on 07/13/18. The file for Staff #2 contains a PA State Police clearance that was printed on the wrong form.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person # --1 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Noncompliance Area: The first aid kit contained non-sterile gauze.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
