Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Description: Building Surfaces

Noncompliance Area: In the main room, there was a mirror mounted on the wall near the couch that had sharp corners that were accessible to children. On the outdoor play space on the porch, the wood lattice on the railing around the porch was broken and coming apart from the railing in multiple areas with sharp edges and had a few exposed nails with sharp edges that were accessible to children.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: According to the log, the operator did not manually test the interconnected fire detection system in the facility at least once every 30 days between 10-10-25 to 11-10-25 and 11-10-25 to 1-10-26.

Noncompliance Area: In the computer room, there was a doll that had the fabric torn and had a hole with the stuffing coming out of the doll. In the computer room, there was a bucket that contained markers on the chest/dresser that had cracks in multiple areas with sharp edges.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Supervised at all times

Noncompliance Area: In the main child care room with staff person #1 supervising 5 children, 1 school-age child was observed leaving the main child care room alone and entering the kitchen to get a drink of water without a staff person present in the kitchen and then returning to the main child care room approximately 15 seconds later.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Description: Staff assigned to specific children

Noncompliance Area: On 3-27-24 at approximately 9:45 a.m., staff person #1 was observed supervising 5 children in the main child care room, including 4 school-age children and 1 preschooler. Staff person #1 was observed leaving the main child care room alone and walking into the kitchen, with the children in the main room alone without a staff person present. Staff person #1 could visibly see the children through the wall cutout between the kitchen and main room. In the main child care room with staff person #1 supervising 5 children, 1 school-age child was observed leaving the main child care room alone and entering the kitchen to get a drink of water without a staff person present in the kitchen and then returning to the main child care room approximately 15 seconds later.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Description: Locked or inaccessible - in original container

Noncompliance Area: In the kitchen hallway area near the door, there were multiple toxics on the window ledge, including Pine-Sol and Windex, that were labeled "Keep out of reach of children" and were accessible to children. In the bathroom on the sink counter, there were 2 bottles of Purell Hand Sanitizer that were labeled "Keep out of reach of children" and were accessible to children. On the left porch, there was an open bag of Road Runner Ice Melt that was labeled "Keep out of reach of children" and was accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Description: Building Surfaces

Noncompliance Area: The exit door on the right porch of the facility had the siding on the surface of the door in the bottom left corner that was coming apart, and the wall to the right of the door had the siding coming apart with a hole in the bottom right corner of the wall near the door. The wooden lattice fence on the railing on the porch had one section of the fence that had come apart and was not attached to the fence. On the outdoor play space, there was a brick on the walkway leading to the back gate that was cracked and broken. On the outdoor play space, the plywood on the side of the left porch wall to the left of the steps was broken in multiple areas with sharp edges and there were 3 screws/nails that were protruding out and were a visible hazard.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Noncompliance Area: The file for Child #1 was lacking one dose of MMR, 1 dose of Varicella, and 1 dose of Hepatitis A.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: The facility was lacking a hands-free covered can at the changing area.

Noncompliance Area: Upon stopping to check smoke detectors on 7/30/20 The smoke detector on the main floor that is interconnected with the one in the basement was not working. The basement one was working.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 and 2-- may not work in a child care position at the facility.

Noncompliance Area: 3 cots kept in the closet were torn and fraying. The cover on the springs of the individual trampoline was torn and peeling.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The file for Child #2, a Young Toddler age child, contained an initial health assessment dated 1/29/18 and an updated health assessment dated 3/6/19, the two being more than 6 moths apart.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: There was a board with a nail sticking out of it on the play area of the facility.

Noncompliance Area: The file for Child #1 was lacking a Health Assessment completed by the doctor that was completed within 60 days of enrollment.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: On 6/15/2017, at approximately 12:20 pm, staff person #1 was observed caring for a group of 9 children consisting of 1 younger aged toddler, 1 older toddler aged child, 1 preschool aged child, and 6 younger school aged child. The staff child ratio for this mixed age group is 1:5.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements at 3280.52(b). The required staff:child ratios must be maintained at all times.

Noncompliance Area: In the outdoor play space multiple nails and sharp edges from building straps were observed to be accessible to the children in care along the edges and joints of the landscaping ties used to border the play equipment.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: The pack and play which child #4 was observed napping in during the inspection on 3/27/2017, was observed not to be labeled for the individual use of that child.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child`s parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: The agreement on file for child #5 was observed not to have been signed by the parent.

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: The file for children #1, #2 and #3 were observed not to have a child service report completed within the last 6 months.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department`s approved form to provide information to the family about the child`s growth and development in the context of the services being provided. Noncompliance Area: The file of child #5 was observed not to have a health report on file.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Noncompliance Area: A health report for child #6 a school aged child was observed not to be on file dated at the time of child #6 entering kindergarten. The most recent health report on file for child #6 was dated 3/20/2014.

Correction Required: The initial health report for a school-age child must be dated in accordance with the requirements for medical examinations for school attendance in 28 Pa. Code 23.2 (relating to medical examinations).

Noncompliance Area: The file for children #2 and #3 were observed not to have health report completed within the last 12 months.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: It was observed that the operator did not establish an individual record for child #4.

Correction Required: An operator shall establish and maintain an individual record for each child enrolled in the facility.

Noncompliance Area: The agreement and emergency contact forms on file for children #1, #2, #3, and #6 were observed not to have been reviewed by the children's parent and documented with a dated signature within the last 6 months.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information. Following review, a parent shall attest to the accuracy of information in the emergency contact information and the financial agreement at each review by affixing a dated signature to the record.

Noncompliance Area: A glass storm door located in the front child care space was observed not to have a visual identification strips in place.

Correction Required: A visual strip or other visual identification shall be placed on glass located in a traffic area, a child care space or a play space.
