Noncompliance Area: The file for Child #1 contained an immunization record that was lacking a dose of the HIB immunization and two doses of the Hepatitis A immunization. The file for Child #2 contained an immunization record that was lacking one dose of DTAP, and two doses of HIB. Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP. |

Provider Response: (Contact the State Licensing Office for more information.) Provider informed parents 9-25-19 of immunization record. Children's pediatrician stated that both children are up to date on their immunizations and appointments were already set up for immunizations in question. Written information from doctor was submitted for Child #1. I will continue to have parents update their health assessments every 6 months as required. Child #1 appointment is 11-25-19, and Child #2 appointment is 10-4-19 |
