Children's pediatrician stated that both children are up to date on their immunizations and appointments were already set up for immunizations in question. Written information from doctor was submitted for Child #1. I will continue to have parents update their health assessments every 6 months as required. Child #1 appointment is 11-25-19, and Child #2 appointment is 10-4-19 |
