Findings: Based on surveyor's review and an interview with the director, it was found that the facility no longer serves infants and the office had moved from the Camel room to the Roadrunner room. Comment: Facility will be submitting a modification submittal to reflect these changes which will change services offered by the facility and the overall capacity.

Child 2 was lacking a health care provider phone number and any medical and nutritional information on the back of the card. Child 3 was lacking a health care provider phone number and child 4 was lacking any medical or nutritional information on the back of the card and a parent's signature.

Findings: Based on surveyor's review and an interview with the director, it was found that in the Meerkat room there were 15 children not signed out of the facility on several days during the month of October. In the Tortoise room child 5 was not signed out on 10/6. In the Sand Cats room child 6 was not signed out on 10/19, child 3 was not signed out 10/21 and child 7 was not signed out 10/4 and 10/5.

Findings: Based on surveyor's review and an interview with the director, it was found that staff 2 began employment 10/24/17 and was lacking documentation of her education. Staff 3 started employment 10/18/17 and her education documentation had not been translated. Reference: R9-5-402.A.

Findings: Based on surveyor's review and an interview with the director, it was found that staff 1 began emplyment 10/2/17 and was lacking a copy of her fingerprint clearance card in her file. The DPS website verified the validity of the card which had issued it on 9/25/17.

Findings: Based on surveyor's observation, it was found that there were packages of antacid, non aspirin, antibiotic ointment and sting relief pads int the facility's first aid kit. On van 1 and van 2, there were packages of burn cream and antibiotic ointment in the first aid kits.

Findings: Based on surveyor's review and an interview with the director, it was found that on van 1 and van 2 the Emergency, Information and Immunization cards were lacking attached immunization records or exemption affidavits. Many of the cards had not been updated with emergency contacts, health care provider information, the name of the person to be called first in an emergency and medical and nutritional information on the back of the card.
