Findings: Based on the Surveyor's observations and staff interview, the licensee was using an unapproved outdoor area for enrolled children. During the inspection, enrolled children were observed playing in the courtyard area outside of the preschool classrooms. A variety of toys and play equipment were observed in the courtyard; including a caterpillar tunnel, a small basketball hoop, a water table, a toddler seating area, and 6 yellow tricycles. Staff stated that this area is used when it is too hot to go to the playground or when a therapist is working with a child inside of the classroom. The outdoor courtyard area is not currently part of licensed space and has not been approved for child care services.

Findings: Based on facility documentation, the Surveyor's observation, and staff interview, it was determined that child #6 was not signed out of the program while attending speech therapy. During the inspection, the Surveyor observed child #6 alone in Room #106 with the speech therapist while child care staff members were outside in the courtyard. A signed permission form was on file in the classroom for the child to be signed in and out for therapy services. Staff stated that they were unaware of the requirement to sign enrolled children out when participating in therapy services that are unsupervised by child care staff.

Findings: Surveyor reviewed 7 staff files and interview with the Director determined that 1 of the 7 staff files lacked documentation verifying that results of a negative Mantoux skin test given to a staff member was within 12 months before the starting date of employment. * Staff #1-starting date of employment-8/14/2018. The file had documentation indicating that a negative Mantoux skin test was read on 8/4/2017. The results were not verified within 12 months before the starting date of employment.

Findings: Based on Surveyor observation and interview with the Director, it was found that the diaper changing surface in the outside Girl's Bathroom was rough to the touch and worn (with ridges) in several areas.
