Correction Required: Competence in first-aid is demonstrated by the completion of training by a professional in the field of first-aid. First-aid training shall be renewed on or before expiration of certification or every 3 years, as applicable.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: During renewal inspection on 5/24/18, cert rep observed first aid kit lacked scissors.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: During the renewal inspection conducted on 10/16/17, Certification rep did not observe a health assessment on file at the facility for the operator. Certification rep pulled a copy of the operator's health assessment from the operator's record at the Southeast Regional office. The health assessment was dated 10/15/15.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment on file at the facility. A facility person`s record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Correction Required: An individual record is required for each facility person.

Noncompliance Area: During the renewal inspection conducted on 10/16/17, Certification rep did not observe a copy of the applicable regulations under which the facility is certified (Chapter 3290) posted in the facility. On 11/17/17, Certification rep still did not observe a copy of the applicable regulations under which the facility is certified.

Correction Required: The facility`s current certificate of registration and a copy of the applicable regulations under which the facility is certified shall be posted in a conspicuous location used by parents, with instructions for contacting the appropriate regional day care office posted at the same location

Noncompliance Area: During the renewal inspection conducted on 10/16/17, Certification rep did not observe verification, in writing, that the operator reviews the facility emergency plan annually.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: During the renewal inspection conducted on 10/16/17, Certification rep did not observe any verification of the operator completing the required minimum hours of child care training. On 11/17/17, Certification rep still did not observe verification of the operator completing the required minimum hours of child care training.

Noncompliance Area: During the renewal inspection conducted on 10/16/17, Certification rep did not observe any of the required clearances (Child Abuse, PA State Police and FBI) on file at the facility for the operator. Certification rep pulled a copy of the operator's clearances from the operator's record at the Southeast Regional office. The Child Abuse clearance was dated 9/22/07. The PA State Police clearance was dated 9/10/07. There was no FBI clearance. Certification rep also did not observe verification that the operator completed the Mandated Reporter training, also required by CPSL. On 11/17/17, Certification rep still did not observe verification that the operator completed the required Mandated Reporter training.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person`s record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.
