Findings: A teacher was left alone to supervise children when the second teacher took a child to another classroom for diapering. The classroom was left in a ratio of 1:17 and the youngest child was three years old.

Findings: Surveyor #1 walked into Classroom 102 and observed Staff #3 standing over a mat where a child was laying. She bent over the child and in a loud and harsh tone told the child to stay on their mat otherwise she would tell on them.

Findings: Staff #3 and Staff #4 were preparing for nap time in Classroom 102. Children were made to lay on mats with their blankets, however, the cots were not covered with sheets. When asked where the sheets were, Staff #3 stated that the sheets were located in an area in the room and was not aware that the cots must be lined with a sheet before children lay down.
