|Findings: Upon review of the children's emergency cards, Surveyor #2 observed that Children #1 - #3 were missing the date of the child's enrollment. Additionally, the "grab and go" binder of emergency cards were all missing the back of the cards with information regarding the nutritional and dietary needs of the child including, the child's susceptibility to illness, physical conditions of which a staff member should be aware, and any individual requirements for health maintenance.|
|Findings: Surveyor #2 observed that the children's immunization records were not attached to their emergency cards.|
|Findings: Upon review of staff files, Surveyor #2 observed the following: a) Staff #1 started working on 5/2/16, but only had 5.5 hours of annual training from 5/2/16 to 5/2/17.|
|Findings: During the inspection, the Surveyors observed a 0:1 ratio in the hallway when Child #4 went to the restroom alone.|
|Findings: In the preschool classroom, the Surveyors observed that the children were napping but it was too dark. The Surveyors could not see anything. This was corrected during the inspection.|
|Findings: Surveyor #1 observed a toilet brush and a plunger in the girl's restroom. This was corrected during the inspection.|
|Findings: During the inspection, the Surveyors observed a lack of supervision in the hallway when Child #4 went to the restroom alone.|
|Findings: The Surveyor found that Staff 1 was designated to act on behalf of the director however Staff 1 is not qualified as required by R9-5-401(2). Staff 1 was lacking the education and work experience.|
|Findings: Surveyor 2 reviewed five emergency cards and found the following: a) Child 1 and Child 2 were missing a number for the health care provider. b) Child 3 was missing the name and number of a health care provider. c) Child 4 was missing a date of enrollment.|
|Findings: Surveyor 2 found the immunization record for Child 3 was not attached to the child's emergency card.|
