Challenge Charter School will provide a sound educational environment grounded in the fundamental skills of a hands-on, science-oriented, academic core curriculum. The environment is structured to optimize each student's opportunities so that all students can acquire a first-class education that will prepare them to be successful in the lifelong learning process. Schools should be expected to create a structure that provides more consistency for students in person, place, and more time on task.

Initial Comments: There were no deficiencies at the time of the Annual Compliance Inspection conducted on 11/4/2025. This report is subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was completed at the time of the inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspections. During the exit interview, the following items were discussed, but not limited to: *Days and Times for Emergency Drills. *TB Attestation. *Updated Annual Training requirements. *Maintenance and cleanliness of outdoor equipment. *Ensure gaps in fencing do not exceed 4" from the ground. *Ensure the capacity is posted in each activity area used by enrolled children.

Initial Comments: There were no deficiencies at the time of the Compliance Inspection conducted on 11/8/2024 but are subject to changes pending programmatic review. The Emergency Disaster Contact Form was completed at the time of the inspection. The Empower Self-Survey was emailed to the director at the time of the inspection. Please complete it within 10 days. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Ensure turf on the playground is maintained and free of gaps. *Ensure all required information is completed on Emergency, Information, and Immunization Record cards. Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 11/15/2023, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The DES Contact form was completed at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Ensure that all gates to outdoor activity areas remain closed. *Ensure that children are not entering unlicensed areas.
