Per the DPS website, the renewal fingerprint clearance card application was not received by DPS until 05/17/2021, leaving a 15 day period of time that the staff member lacked a valid fingerprint clearance card. * Previously cited 01/16/2019.

Findings: Based on facility documentation and the Surveyors' observations, the licensee lacked documentation of a fire drill conducted during the month of July 2021.

Findings: Per allegation and based on staff interview, facility documentation, and video review, it was determined that on 06-10-2021, staff #1 handled an infant in a rough manner while the infant was in a crib. Staff stated that the staff member "seemed agitated" because the infant was crying. Per video review of this incident by the Deputy Bureau Chief and Surveyor, the following occurred: The infant was laying in the crib on their tummy and was moving around. Staff #1 walked over to the crib, abruptly grabbed the infant under the arms, quickly spun the infant around, flung the infant into the back corner of the wooden crib, and the infant dropped onto the mattress. After this, the staff member turned and began to walk away from the crib. The staff member stopped for several seconds, turned back around and walked over to the crib again, grabbed the infant, and quickly flipped the infant over onto their back. The staff member stood there for a second, then quickly flipped the infant back over on their tummy. The aggressive actions of staff #1 resulted in the infant sustaining a head injury. The director, assistant director, and District Manager confirmed that the infant had been crying throughout the entire incident.

Findings: Per allegation and based on staff interview, it was determined that staff #2 left an enrolled 3-year-old child alone and unsupervised in the restroom during a facility fire drill. Staff stated that during the fire drill, staff #2 and enrolled children from the 3's classroom were in the process of evacuating the facility. When the staff member arrived at the back hallway exit door, staff stated "I don't have all of my children". Another staff member walked into the 3's classroom and observed the child walking out of the restroom door. * Previously cited 01/14/2020.

Findings: Based on facility documentation and the Surveyors' observations, it was determined that the 1's classroom lacked a current lesson plan; the posted lesson plan was dated July 26th. The 3's classroom lacked a current lesson plan; the posted lesson plan was dated August 2nd. * Previously cited 01/16/2019, 01/14/2021, and 07/22/2021.

Findings: Based on the Surveyors' observations, in the infants' room, 2 crib sheets were not fitted to the mattresses. The sheets were too tight on the mattresses and caused the mattresses to bow upward away from the crib sides.

Findings: Based on staff interview, staff stated the following: * Sometimes staff spray enrolled 2-year-old children with a water bottle (water only) to get the children's attention and attempt to make them stop doing something they should not be doing. * The water bottle is only used while the staff is changing an enrolled child's diaper and they are the only staff member in the room. * Other methods of discipline have not worked effectively, so they use a water spray bottle and it tends to make the children stop the undesired behavior.

Findings: Per allegation and based on staff interview, it was determined that staff #3 (while in the 2's classroom) has used a method of discipline that could cause harm to the health, safety, or welfare of an enrolled child. Staff stated the following regarding staff #3: * She would get frustrated easily. * I saw her pick up a child roughly by one arm, I informed the director. * She did pick up a child in a rough manner. A child was standing on a table, and she grabbed the child off the table by one arm and moved them to the floor. It was during the first part of June. I saw her do this and brought it to the attention of the director. I have also seen her grab and pull children in a rough manner. She was just really rough with the kids. * A teacher told me that staff #3 picked up a child by one arm.

Findings: Per allegation and based on staff interview, it was determined that staff #3 and staff #5 have both used abusive language while speaking to enrolled children. Staff stated the following: Staff #5 told an enrolled child, "You better stop that you little shit or I will knock you out!" Staff #3 told an enrolled child, "You need to stop that you ugly little rat!"
