Noncompliance Area: The most current emergency contact information on file for child #1 lacked the telephone number of all individuals designated by the parent to whom the child may be released.

Noncompliance Area: The most current health assessment on file for child #1, who was enrolled on 12/8/2019, lacks an initial health report within 60 days of enrollment. There is no health assessment on file.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.A child's record must contain initial and subsequent health reports.

Noncompliance Area: The most current immunization record on file for child #2 lacks verification of a flu shot within the past year as in accordance with the schedule recommended by the ACIP.

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: Based on a conversation with the operator and medication log on file, child #4's parent brings a prescription medication for the child every day. Parent brings one pill in a baggie which is not in the prescription container in which it was received. Staff #1 has been administering the medication and written instructions were not provided from the individual who prescribed the medicine.

Correction Required: A prescription or nonprescription medication may be accepted only in an original container. The medication shall remain in the container in which it was received. A staff person shall administer a prescription medication only if written instructions are provided from the individual who prescribed the medicine. Instructions for administration contained on a prescription label are acceptable.

Noncompliance Area: Observed a medication log for child #4. Written consent for administration of medication was not present in the log.

Correction Required: A parent shall provide written consent for administration of medication or a special diet.

Noncompliance Area: The files for staff #1 and 2 lacked a current health assessment which was conducted within the past 24 months.

Correction Required: A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. The operator shall document in writing each review and update of the emergency plan.

Noncompliance Area: The file for staff #2 lacked verification of training regarding the emergency plan within the past year.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training. A facility person's record shall include records of training required by the Department.

Noncompliance Area: The files for staff #1 and 2, who worked in a child care position prior to 9/30/2019, lacked verification of a National Sex Offender Registry certificate by 7/1/2020. There are now certificates on file and dated 7/8/2020.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: In the kitchen, observed 3 first aid kits that were sitting on the floor beside the refrigerator and in reach of children.

Correction Required: A first-aid kit shall be inaccessible to children.

Noncompliance Area: In the toy room, observed a child's play crib which was partially blocking the back door which leads to the outside deck.

Correction Required: Stairways, hallways, exits from rooms, exits from the facility and other means of egress serving as an exit shall be unobstructed.
