Provider Response: (Contact the State Licensing Office for more information.) Since I have been licensed in 2000, I have never had to keep all health forms on file from the previous years, when a representative comes out each year and checks they are up to date, however now I am required to keep them from the time of initial enrollment, so in the future I will keep all initial and subsequent health forms in each child's file. As discussed with State Inspectors. |
