Initial Comments: The following deficiencies were observed at the time of the annual compliance inspection conducted on 12-05-2025, and are subject to change pending programmatic review. 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group size was compliant at the time of the inspection. The following was discussed, but not limited to: 1. Criminal History Affidavit documentation. 2. TB attestation requirements. 3. 10-day new staff orientation training requirements. 4. Annual staff training hour requirements. 5. Good-faith effort reference requirements. 6. Menu documentation requirements. 7. Food items in the diaper changing area(s). 8. Toys/equipment maintenance. 9. Storage of cleaning equipment. 10. Stacked items. 11. Storage of food waste. 12. Diaper-changing sink temperatures.

The fingerprint clearance cards for 7 of 7 staff members were verified to be valid through the DPS website at the time of the inspection. Initial Comments: The purpose of the inspection was to conduct Complaint #00059678 investigation on 6/5/2023. The following deficiencies are subject to changes pending programmatic review. The Plan of Corrections will not be accepted at this time. A full inspection was not conducted. Ratios observed were: Infants- 2:6 1's- 1:6 2’s- 2:13 3's/4’s- 2:16 5’s- 1:13 School-Age- 1:12 There were 3 staff interviewed during this investigation. There was 1 staff file reviewed during this investigation. There were 2 child files reviewed during this investigation. There were 3 children interviewed during this investigation. Others interviewed: Director, Complainant. Documentation observed was: Written Incident/ Accident Reports, Staff and Child Attendance Rosters, Sign In/Out Records. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that one of two allegations was substantiated, and the other allegation lacked sufficient evidence to be substantiated.
