Initial Comments: There were no deficiencies observed at the time of the Compliance Inspection conducted on 4/14/26 and are subject to changes pending programmatic review. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The fingerprint clearance cards for 2 staff members were verified to be valid through the DPS website at the time of the inspection. Items discussed but not limited to: fire drill evacuation times

Initial Comments: The following deficiencies were found at the time of the compliance inspection conducted on 4/17/25, and are subject to changes pending programmatic review. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Empower Survey link was emailed to the facility. The DES group size was observed at the time of the inspection. Please complete the Plan of Corrections via the online portal within 10 days of receipt of the Statement of Deficiencies. Send the current gas and fire inspections within 10 days of receipt of the statement of deficiencies.

A full inspection was conducted at this time. Three of three fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.
