Findings: * Surveyor 1 reviewed the personnel file for Staff 1 who was working in the infant room as an Assistant Teacher-Caregiver. Staff 1 had a diploma from a foreign country that had not been evaluated to determine that is equivalent to a high school diploma in the United States. Staff 1 did not have any documented child care experience.

Findings: * Surveyor 1 observed an infant lying on top of a large blanket that had an elephant pillow attached to it. There were other toys and a thick winter hat within the child's reach. The infant was lying on their back while on the pillow blanket. (a picture was taken)

Findings: * Surveyor 1 observed that the drinking fountain on the small playground next to the infant room was not working. The drinking fountain was not clean; there was dirt and twigs from the trees sitting in the sink. (a picture was taken)

Findings: * Surveyor reviewed the infant daily logs for Child 1 that are given to the parents. The facility did not have a daily log for 10/14/21. * Surveyor observed that the logs dated 10/5/21 and 10/11/21, did not include the activities the infant participated in. * Surveyor also observed that the logs dated 10/8/21, and 10/12/21, did not have any diaper changes logged.

Findings: * Upon entering the school-age classroom, Surveyor counted 22 children with only 1 staff. * School-Age children were allowed to go into the older 2's classroom to get a drink of water while on the playground; which put that classroom over ratio, as there were already 2 staff with 16 two-year-old children.

Findings: * Upon entering the school-age classroom, Surveyor heard a child screaming loudly and crying. The child had a paper towel placed over their forehead. Surveyor asked the Staff why the child was crying. She stated that they had been outside and she did not have a water pitcher for the children. The child stated that they were thirsty. The child vomited during the inspection on the floor next to several other children after telling staff that they were thirsty.

Findings: * According to the staff in the school-age classroom the children were playing outside, but she did not have the water pitcher. She stated that if the children were thirsty they had to go into the two-year-old classroom to get a drink of water. * Surveyor spoke to the staff in two-year-old classroom who verified that they saw at least three children enter their classroom to get a drink of water. * There were 22 children in the school-age group.

Findings: * Upon entering the infant classroom, Surveyor observed 3 infants in feeding chairs. Surveyor observed that each of the feeding chairs were equipped with a safety strap. However, none of the safety straps were being utilized.

Findings: * Based on an interview with staff from the infant room, they stated that they occasionally feed Child 1 table food provided from the kitchen. * Upon review of the feeding instructions for Child 1, there is no written permission to feed the child table food.

Findings: Per Allegation * According to the Director, the staff in the infant room complete the infant daily logs as well as a separate diaper changing log for each of the enrolled infants. * According to the staff in the infant room they document the diaper changes on the diaper changing log first then transfer it to the infant daily log. * Surveyor observed that the infant daily logs and diaper logs for Child 1 had some discrepancies. The infant daily log dated 10/8/21, did not document any diaper changes. The diaper log dated 10/8/21, indicated 7 diaper changes including the last one at 4:30. However, according to the child's attendance record, they were signed out of the facility at 4:13. * The infant daily log dated 10/11/21, documented 6 diaper changes. The diaper changing log documented 5 diaper changes; none of times matched on either of the documents. * Previously cited on 1/7/20 regarding a complaint investigation.

Findings: * Surveyor 2 reviewed the fire drill log and observed that a fire drill was conducted on August 16, 2020; which was a Sunday. The facility is not open on the weekends.

Findings: * Surveyor 1 observed that the posted capacity in the Young One-Year-Old Classroom was 10. The correct capacity is 11.

I have used this provider for more than 6 months

I have heard alot of complaints from my children about the teachers yelling, being mean, and favoring certain children. Well, just today as I was picking up my children I walked by an INFANT room. The window was open and I heard a baby cry, and the teacher said "No, your annoying!". As a parent I became FURIOUS at the thought of someone who is to care for OUR children, speaking to a baby like that. Know I believe my kids, and I can bet she abuses the babies because no one is watching and they can't vocalize yet. I am honestly looking for another provider. Don't take your babies here!

I have used this provider for more than 6 months

There horrible staff, Taken money from my card when I don’t even owe them. I talk to kelsey the owner and she’s very rude looks like she in something. And one of the teachers told one of the kids to shut up she was barley like 3 years old and I went to talk to kelsey about it and still very rude. I hope they end up closing that childcare is horrible

I have used this provider for more than 6 months

My kids went there for a couple years and I thought it was wonderful until I became a staff there and saw it for what it really was. And when I quit they disenrolled my kids without mentioning it to me.. Just because I quit. The 'school' is not what it appears to be. I was very disappointed. It is also infested with mice and coackroaches.
