Noncompliance Area: The Fire Drill and Fire Detection log was observed for the 30-day smoke detector checks. It was observed that there were 31 days between 5/3/25 to 6/3/25, 36 days between 6/3/25 and 7/9/25, 36 days between 7/9/25 and 8/14/25, 43 days between 8/14/25 and 9/26/25, 43 days between 10/8/25 and 11/20/25, 32 days between 11/20/25 and 12/22/25, and 40 days between 2/13/26 and 3/25/26.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Noncompliance Area: The file for staff person #1 lacked a health assessment and a Tuberculosis screening by the Mantoux method at initial employment.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment on file at the facility. An adult health assessment shall include Tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, CRNP, the Department of Health or a local health department.

Noncompliance Area: In the kitchen two silverware drawers to the left of the entrance to the kitchen from the front door were observed to have sharp knives posing a threat to the health and safety of the children.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: In the kitchen a can of Hot Shot flying insect killer, a bottle of Fantastik spray, a bottle of Whip-It stain remover, and a bottle of OXY deep carpet cleaner all labeled "Keep Out of Reach of Children" were in a low corner cupboard accessible to children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.
