I have not used Little Tribe Day Care in Washington Ok. I live next to Little Tribe Day Care. We have been turned in twice. Once for having our Grandchildren here during COVID, we have 7 ages 2 to 14. The 2nd one was just this week. We have a handicapped son, 43. He is unable to speak and can only make sounds. He loves people and he loves church. He thinks the day care is a church because it is was a church and looks like a church. He was trying to talk to them. I would like to say when they remodeled the outside they had to use our water and hose several times and we never minded. We also get many of their packages and we always take them to them. I was very surprised this little day care called the police at our house with trespassing paper work for a developed mentally delayed person. Why didn’t they come tell us he came over, like they would walk over and ask to use our water. Again I’ve never used them, just a neighbor. Our son functions around an 7 year old level.

Description: Licensing observed a personnel hired on 2/16/2026, that does not have ELCCT training and four (4) personnel that has not completed infection control training since 9/21/2024. Two (2) new employees were employed on 2/16/2026 & in March 2026 and personnel information form's were not submitted to Licensing within 2 weeks of employment. Licensing observed a one-year-old in a crib asleep, laying on top of two (2) mattresses, the sheet was only over the corners on the top mattress, and the top mattress was pulled up away from the bottom mattress. Licensing observed a large hole in the ceiling where a tile is missing and insulation can be seen. |

Licensing observed an infant asleep in a swing. Licensing asked personnel how long the infant had been asleep in the swing and personnel said around 20 minutes. Licensing observed an infant asleep in the crib with a bottle in the crib. |

Description: Licensing observed program's last fire inspection was conducted on 1/27/22. (Serious) Licensing observed an infant asleep in a crib with a pacifier. Infant was asleep on stomach and pacifier was not in the infants mouth. Licensing picked up pacifier and pacifier was attached to the infant's clothing. (Serious) Licensing observed one personnel working without a submitted back ground to OBI. Personnel has been employed since 2/15/24. Licensing observed in the kitchen a "Food Anaphylaxis Action Plan" that had been submitted by parent from the doctor's office. Licensing asked director to view medication permission form for child and director did not have one on file. Licensing observed in the hallway were cubbies are located a missing ceiling tile with hanging light fixture and insulation exposed. Another room not being used at the time ceiling tile was missing with lighting and insulation exposed. Licensing observed 3 personnel with expired PDL's. Licensing checked with CECPD regarding personnel PDL's. HH PDL expired on 3/2/24, no application has been submitted for renewal and currently has 2 hours of training. MH PDL expired on 10/21/23, no application has been submitted for renewal, transcript was submitted and CECPD received it in Dec of 2023. EP PDL expired 11/18/23, no application has been submitted for renewal, no training has been completed or training sent for review since 2022. Licensing observed that the program currently does not have a Qualified Master Teacher. (Serious) When licensing entered the first room in the program personnel left to get director. The second room licensing entered was the room director stated she was currently overseeing. Director continued with licensing, while licensing walked to 3 other classrooms with children left unattended in the second room. |
