I have used this provider for more than 6 months

My son has been enrolled in daycare for a couple years and he loves it! It is not one of these new fancy daycares but personally i like that. Its more homely and small so u get to know everyone! He cries somwtimes in the morning when i leave which is normal for him. But he has a great day! Hes comfortable with all the teachers he know the routine and loves his friends. My daughter has been to the afterschool program when they had it before corona and honestly was mad if i picked her up early. She goes to the summer camp and they ALWAYS have something scheduled to entertain the kids and she has a great time. I highly recommend!!!!

First Assembly Daycare was established in 1980 to care and nurture children in the love of God. We have cared for thousands of children and have been able to share the love of Jesus with them and their parents.
We currently offer weekday care for children ages 6 weeks through 4 years.

In the room serving four (4) year old's there was a file cabinet with one low drawer that would not shut.

Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection that was an approved report was in 2021. They facility had an inspection in October 2022, but they did not pass this inspection. They had a reinspection in January 2023 and did not pass that inspection. An approved inspection must be obtained within 14 days of today's visit.

In the classroom that routinely serves children who are two years old, there was both a one-year-old and a three-year-old enrolled and present. Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. At the gate going into the playground that they younger children play on there was a gap at the bottom of the gate due to erosion of more than 6 inches but less that nine inches. This area would allow a child to crawl under and leave the playground.

On the playground where the older children were located there was a cap of the fence post leaving the top exposed and it was sharp.

Lavatories were not kept clean, in good repair and kept free of storage. The bathrooms in the Preschool building were not clean around the bottoms of the toilets as evidenced by yellow liquid around the bottom of the toilets. There was pain chips and rust on bathroom partitions. In the bathroom in the mobile unit used for the Infant and Toddler Children there was boxes stored on the floor and the wall was not in good repair.

All walls and ceilings including doors and windows were not kept clean and in good repair. The walls in the Preschool building in Space 1 were in need of repair. There was ceiling damage in kitchen, lobby and classroom where the preschool children were located. There was wall damage & rusty partitions in hall bathrooms in Preschool Building. I observed a need for improved cleaning on the lower parts of the walls in the bathroom areas.

All equipment and furnishings were not in good repair. In the classroom where the 2 year old children were located there was a table that had a lament top and it was chipped on the corners.

A safe indoor and outdoor environment was not provided for the children. On the Infant and Toddler playground area there was damage to the Gym wall where the metal siding was rusted and broken. Beside this same wall there was an area that had been effected by drainage from the top of the building where it has washed away some of the dirt area exposing small rocks which were small enough to be choking hazards and the landscape in this area did not support new walkers and could cause the children to trip or fall. The black drainage pipe from the side of the building was broken and jagged and the area that was exposed had rocks placed in it keeping the drainage pipe from effectively draining the excess water from the top of the building. There were areas in the fence that allowed for small children to crawl out or into unsafe areas at the gate exit and at the fenced air condition area. On the handicapped ramp that is used to enter and exit the mobile unit where the infant, toddler and two year old children's classrooms are there were more than 15 nails that were risen up and needed to be pounded back down in order to ensure that there is safety hazards.

The fence met the minimum height but was not free from protrusions as evidenced by a top fence pole sticking six inches out from the fence and having no cap on the pole leaving the edge sharp and unsafe.

Parent's medication authorization did not include required information. On many of the permission slips to administered bug spray there was no information listed except for the parents signature. On other authorization forms there was not enough information listed to provide the staff with the needed information to apply.

Upon review of the staff and training worksheet and staff files it was observed that all but one staff member had not updated their emergency information within the last year.
