A safe indoor and outdoor environment was not provided for the children. In the outdoor learning environment there was a soccer goal that had metal poles that were disconnected at the bottom leaving jagged edges that could be a safety hazard if touched. This goal was accessible to the children in care.

All stationary equipment, more than 18 inches high, was not installed over protective surfacing. In the outdoor learning environment there were three different structures that were installed over some surfacing but it did not meet the depth requirement to be considered protective surfacing. The mulch measured anywhere from 1 to 4 inches at the end of the yellow and red slides and two inches at the under the climber.

All equipment and furnishings were not in good repair. The yellow slide on the back side of the play equipment had a crack in the slide. Although the Program Coordinator stated it had been identified and previously secured so that children could not play on this slide today there was no such hazard protection.
