The materials and equipment indoors and outdoors were not sufficent to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. There were no age-appropriate materials accessible to the children in this classroom there was three toys with empty shelving and three tables.

In all classrooms that serve children ages 2 through 12 years of age, with the exception of the classroom that serves children 3 years of age, there was not the required materials that would allow for children to play with three activities simultaneously or independently.

All floors and floor coverings were not constructed of nonabsorbent, easily cleanable material. In the space where the one-year-old children were located there was molding that had come unattached from the wall area and needed to be replaced in order to be an easily cleanable area.

In the classroom serving two-year-old children, a new air conditioner had been put in but the material blocking the space in the window that wasn't taken up by the unit had been pushed out and this made an opening to the outer air.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the space were the one-year-old children were located there was a cabinet with a lock on its door but it was not locked and inside the cabinet was a small aerosol can of air freshener.

New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. There were two staff persons file reviewed and both staff orientation log had either no dates for the trainings or the dates were incorrect therefore there was no way to verify if the staff had received the 16 required orientation hours.

At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. The one staff in the classroom with Infants had not taken the ITS-SIDS training and there were no other staff in the classroom with the required training.

The depth of the loose surfacing was not based on critical height of the equipment. The playground equipment has compacted mulch and the mulch at the base of the slides is so compacted that there is no protective surfacing.

Screen time was offered to children under three years of age. In the classroom serving children who were two years of age the children were sitting in their chairs watching Zootopia waiting on lunch.

Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. In two of the classrooms the children were sitting over five (5) minutes waiting on lunch to be served with no activities for the children to do. The staff, MM, was verbally assertive trying to keep the children from playing with the toys and asking them to sit and do nothing while waiting on lunch.
