Neither of the cans were labeled and were accessible to the enrolled children. * Surveyor 1 observed a large bottle of Rocky Mountain Sunscreen, who according to the Director belongs to the facility, and is applied on the enrolled children. The facility did not have written permission from the parents to apply that specific sunscreen.

Findings: * Surveyor 1 observed that vehicle CD-11582 had wrappers throughout the base of the floor. In back of vehicle, Surveyor observed an open crate with a plastic liner. The crate was overfilled with trash and food wrappers. Hot Cheetos and other chips were crumbled on the floor. The bin holding the blankets and first aid kit had a sucker on the lid that had melted and was very sticky. The floor was sticky with what appeared to be a brown liquid that spilled on the floor.

Findings: * Surveyor 1 reviewed the field trip form dated October 13, 2021 to Brandi Fenton Park. The parent for Child 5 signed the permission form with a first name and last initial only.

Findings: * Upon arriving to the facility, Surveyors observed the staff and children returning from a field trip. Surveyor 1 observed that the children were not wearing identification stating the facility's name, address and phone number. According to the Director, the children were not wearing any of the required information.
