Findings: Per the allegation made in Complaint #AZ00174020, based on staff interviews and review of facility documentation, it was determined that the facility had not obtained written instructions for providing services for Child 1 from their parents. Facility staff reported that they were aware of Child 1 having special needs. Child 1 was enrolled at the facility from 3/1/2021 - 6/9/2021.

Findings: The water temperature in the hand washing sinks next to the diaper changing tables in the 1's and 2's activity areas measured 80. Documentation to verify that the water temperature had been adjusted was submitted to Surveyor 1 on 2/2/2021 and was approved.
