Findings: Based on review of staff files, as well as staff statements, Surveyor 2 determined that none of the three staff members present when Surveyors arrived could provide documentation of current CPR and First Aid. There were eighteen children onsite at that time.

Findings: Surveyor 2 noted that Child 1's Emergency, Information and Immunization Record (EIIR) card did not document two emergency contacts as required, it stated "911".

Findings: Surveyor 1 noted that the child roster on the playground indicated that seven children were present, but there were eight children outside at that time. *The deficiency was corrected during the inspection*

Findings: Surveyor 1 observed the following during the Compliance Inspection: -There was one staff member with eight enrolled children on the playground, the youngest of which was a one-year-old. -Child 4 was alone in the School Age Classroom, causing a staff-to-child ratio of 0:1. The child was engaged in an online class via their electronic tablet while Surveyors were onsite. -Surveyor 1 discovered Child 3 alone on the playground at approximately 9:43 AM, causing a staff-to-child ratio of 0:1. The child's class had been on the playground, but returned to the One's/Older Classroom, leaving Child 3 behind.

Findings: Based on review of required documents, as well as staff statements, the Surveyors determined that neither the Director nor the Director Designee were present when the Surveyors arrived at the facility. There were eighteen enrolled children onsite at that time.

Findings: The Surveyors observed the following during the Compliance Inspection: -The health and safety of a child was put at risk when Child 3 was left alone on the playground after the child's class returned to their One's/Older Classroom. Upon discovering Child 3, Surveyor 1 walked the child into the building and to their classroom. -There was a small trampoline in the Three/Older Classroom.

Findings: The surveyors observed the following during the Compliance Inspection: -Child 3 was left unsupervised on the playground when the child's class returned to their One's/Older Classroom. -Child 4 was alone in the School Age Classroom while participating in an online class on their tablet.

Findings: Surveyor 1 noted that the Activity Schedule posted in Classroom 4 only covered the hours of 8:30 AM to 2:15 PM. According to staff statements, the classroom is often open until the facility closes at 6:00 PM.

Findings: Surveyor 1 noted that the transition between the backsplash and the diapering sink was not seamless in the diaper changing area located in Classroom 2.
