Findings: Upon arriving at the facility the Surveyor and Team Leader were informed that the Director was no longer working at the preschool program. The facility failed to notify the Department of the Director change as required.

Additionally, Child 5 was not signed out on 07/05/2021.

Findings: Upon arriving at the facility the Surveyor and Team Leader observed 1 staff and 5 children, ages two and older, in Classroom 4 (3/4's Classroom). Based on observation and staff statements, it was determined that Child 6 was wearing a pull-up diaper, but an approved diaper-changing area is not available in Classroom 4. *The children and staff moved to Classroom 2, which has an approved diaper changing area, while the Department was onsite*

Findings: Based on inspection of the East playground the Surveyor noted the following items that were in need of repair or removal: -A wooden planter box had splintered wood and exposed screws that were accessible to enrolled children. -A child-sized plastic table that was broken on the top surface. -Several long metal stakes that are used to support the plastic wood chips barrier had worked their way up above the surface of the barrier. -The rubberized coating on the edges of the steel steps of the climbing structure has begun to peel off, causing the bare metal to start rusting. Additionally, the artificial turf lining the center portion of the steps is deteriorating and pulling up, causing a tripping hazard. -The welds on a wrought iron fence have broken, leaving sharp edges exposed. The wrought iron fence is separating a fallen tree and other debris from the playground.

Findings: The Surveyor noted that the blue shade material on the West playground had been damaged by a recent storm, and was no longer able to provide shade. *The Department received pictures documenting the removal of the torn shade material on 07/13/2021*

I have used this provider for more than 6 months

My children have been coming to this daycare for many years!!! They love it and have never been in grave danger. I believe they deserve more of a chance to show people that situation was false! Come on folks lets this facility a break!

Horrible daycare do NOT bring your child here. A child was bitten 25+ times and left with horrific wounds. The staff never notified the parents when it happened or when they picked their child up. If I could post the pictures I would
