Insurance: expires on 5/21/2025, State Fire Marshal: inspection on 10/31/2023, Pima County Health Department: Kitchen permit, expires on 7/31/2025, Items discussed, but not limited to, were: -Supervision of children, -Admission and release signatures, -Staff training hours and dcoumentation, -Review all staff resident files for expiring fingerprint cards.

Initial Comments: The following deficiencies were found at the time of the compliance inspection conducted on 11/2/23, and are subject to changes pending programmatic review. Compliance Officer #1: Katie Corrow Compliance Officer #2: Christine Fiore 4 of 4 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Empower Survey link was emailed to the facility. The DES group size was observed at the time of the inspection. Initial Comments: The purpose of the inspection was to conduct a complaint investigation for case #63772 on 9/15/23. A full inspection was not conducted at this time. Compliance Officer: Katie Corrow Ratios observed were: 3's: 1:2 4's: 5:2 There were 5 staff members interviewed during this investigation. There were 4 children interviewed during this investigation. There was 1 staff file reviewed during this investigation. Others interviewed: Detective from Tucson Police Department. Upon completion of the complaint investigation, it was determined from interview and documentation that the allegation was unsubstantiated. The following deficiencies were observed and are subject to changes pending programmatic review. Please complete the plan of correction via the online portal within 10 days of receipt of the Statement of Deficiencies.
