2025-10-29 Rule states "A child care worker shall record any evidence of unusual bruises, contusions, lacerations, or burns seen on a child, regardless of whether received while in the care of the center". On this date, this Licensor noticed a young child with a long, reddened scratch under his/her right eye. The child reportedly received the scratch when out of care but the injury wasn't recorded in the medical log book. Violation
