Description: Families can come and see what the Sierra Montana Recreation Center has to offer. The center has a game room with Xbox 360s on high definition TV's, ping pong tables, bumper pool tables, a recording studio, pool tables and more! "Game" Tournaments are held frequently. Other activities include outdoor sports, arts and crafts, dance classes, movie nights and more. Register on your first visit, proof of residency and age required. Findings: Based on surveyor 1's observation, it was found that the facility had all doors locked at the time of surveyor 1's arrival. Surveyor 1 walked around the entire building trying all doors and then to the building next door which was a gymnasium/multipurpose room for the elementary school next door. Those doors were locked and the children inside were too loud to hear surveyor 1 knocking. Surveyor 1 then returned to the car and drove to the back of the elementary school. There was a special needs teacher putting children from the school onto a bus. The staff stated that the Recreation program does have a preschool and that they should be there. Surveyor 1 then drove back to the facility and again tried several doors. Surveyor 1 went through the gated playground and knocked on 2 other doors. Children were heard inside, but rooms looked empty from the windows. After approximately 10 more minutes of knocking on a double door, a staff came to the door. Surveyor 1 then was escorted to the only classroom in operation. Options for immediate access were discussed with the director. As the facility often operates an "only" classroom in the large building, safety of the children was of utmost importance. The inside doors have bars to let children out in an emergency, but the outside doors are kept locked. A doorbell is almost non observant from the playground area under a strong sun. Surveyor 1 and the director decided that signs should be posted on all doors in the front of the building with arrows to direct visitors to the classroom. The doorbell should be accentuated to be more visual and tested regularly to ensure visitors are attended to immediately. Surveyor was not able to access the facility for over 30 minutes.

Findings: Based on surveyor 1's review and an interview with the director, it was found that staff 1 was lacking a copy of his fingerprint clearance card in his file. Staff 1 had an application that was not dated but surveyor 1 was able to check that it was valid on the DPS website. The clearance card was issued on 5/29/18.

Findings: Based on surveyor 1's review and an interview with the director, it was found that the snack menu for September listed a cheese stick and water on one day, grapes and water on one day and fruit snack and water on another day. These snacks do not meet the requirements for a second component being served.
