Findings: Based on surveyor's review and an interview with the director and staff, it was found that child 1's Emergency, Information and Immunization Record was completed by the parent and did not state that the child was allergic to pineapple. Staff 1 was initially told by the parent in February with staff 1 informing staff 2. Staff 1 documented the allergy on the posted list in the classroom. Staff 1 inadvertantly sent the lunch served on 7/31/19 jointly to all parents through her tablet and did not send an adjusted one to child 1's parent. The child had bananas instead of pineapple that day. Neither staff in the one's room alerted the director or assistant director about the child's allergy and it was not documented on child 1's record.

Findings: Based on surveyor's review and an interview with the director, it was found that staff 1 began employment on 6/3/19 and had a fingerprint application dated 5/31/19. Staff 1 did not have a fingerprint clearance card in her file and in checking the DPS website, it stated that no application was found. The director stated that according to staff 1, she had mailed the application through UPS. Surveyor instructed the director to send the receipt proved the date the application was found. By 7/26/19, no receipt was received. Surveyor stated that staff 1 could not come back to work until this issue had been taken care of and to be checking the DPS website for updates. Findings: Based on surveyor's review and an interview with the assistant director, it was found that child 1 was not signed out on 7/17 and 7/18. Child 2 was not signed out on 7/17. Child 3 was not signed out on 7/11. Child 4 was lacking a time of departure on 7/2, 7/18 and 7/19. Child 5 was not signed out on 7/11 and 7/12. Child 6 was not signed out on 7/5 and was not signed in, only out on 7/8, 7/12 and 7/15.
